What we collect
When you send a form on this website we collect what you type: your name, phone, email, who you are asking for, the type of care you are interested in, your timeline, how you expect to pay, your preferred tour time, how you heard about us and your message. We ask about care type only. Please do not send medical details through the website.
We also record how you found us: the first page you landed on, the page you sent the form from, the website that referred you, advertising campaign tags if there were any, and whether you were on a phone, tablet or computer. A small first-party cookie named ucc_ft holds this for up to 90 days so it is still available when you send a form.
How we use it
To answer you, schedule tours, and understand which of our marketing is useful. Form submissions are stored in this website’s own database and emailed to our community staff. We do not sell your information and we do not share it with referral services.
Analytics
We use Google Analytics to count visits and actions such as form submissions, phone taps and direction requests. Google’s own privacy policy describes how it handles that data.
